29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: (a) Employees were not provided with locks or tags to attach to the energy isolating devices, prior to conducting the end-of-shift cleaning inside the Daewoo DMV-4020 Vertical Machining Center. Jayna, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1947(c)(5)(i), which was contained in OSHA inspection number 315678821, Citation Number 1, Item Number 2, and was affirmed as a final order on October 22, 2012, with respect to a workplace located at 15 Marybill Drive, Troy, Ohio 45373. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: (a) Employee training on the energy control program was inadequate, in that it did not include training employees on when and how to lock out the Daewoo DMV-4020 Vertical Machining Center prior to conducting the end-of-shift cleaning inside the Daewoo DMV-4020 Vertical Machining Center. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Employees cleaned the inside of the Daewoo DMV-4020 Vertical Machining Center, at the end of the shift, without first isolating the energy sources.
