29 CFR 1910.253(b)(1)(ii): Compressed gas cylinders shall be legibly marked, for the purpose of identifying the gas content, with either the chemical or the trade name of the gas. Such marking shall be by means of stenciling, stamping, or labeling, and shall not be readily removable. Whenever practical, the marking shall be located on the shoulder of the cylinder:   On or about 02/06/13, at 1 La Puntilla Street, Room 125, San Juan, PR, in the flammable storage area, a compressed gas cylinder was accepted for use and did not legibly identify its content by name.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS NOT REQUIRED FOR THIS VIOLATION.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:   a) On or about 02/06/13, at 1 La Puntilla Street, Room 125, San Juan, PR, in the Petroleum room area, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle.   b) On or about 02/06/13, at 1 La Puntilla Street, Room 125, San Juan, PR, in the Petroleum room area, an uninterruptible power supply was not connected to a permanently installed branch circuit receptacle.   c) On or about 02/06/13, at 1 La Puntilla Street, Room 125, San Juan, PR, in the Petroleum room area, a ground-fault circuit interrupter was not installed in all 125-volt, single phase, 15-and 20-ampere receptacles installed within 6 feet or 1.8 meters of the outside edge of a sink.   d) On or about 02/06/13, at 1 La Puntilla Street, Room 125, San Juan, PR, in the wet chemistry room area, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident:  On or about 02/06/13, at 1 La Puntilla, Room 125, San Juan, PR, the circuit breakers or fuses in the electrical panel, that service the laboratory lounge and the  laboratory A/C unit, were not identified or labeled to show the locations of the building whose circuits they protect.
