29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    On or about February 6, 2013, employee was exposed to electrical hazards when the extension cord used to provide electricity to the compressor was plugged into an electrical outlet in a nearby house that was not GFCI protected.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     On or about February 6, 2013, employees were exposed to fall hazards of approximately 9.8-28 feet when the employees were working on a roof installing plywood sheathing and fascia boards with no fall protection.
