29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a. At the site, employees were exposed to a five feet fall hazard, in that, a tubular welded frame scaffold system consisting of 26 frames wide and one frame high did not have the working platform fully planked. Three planks were used and employees were working off a single plank and the other two planks had cement blocks stacked ready for use during cement block wall construction.
