29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a. At the site, employees were exposed to a five feet fall hazard, in that, a tubular welded frame scaffold system consisting of 26 frames wide and one frame high did not have the working platform fully planked. Three planks were used and employees were working off a single plank and the other two planks had cement blocks stacked ready for use during cement block wall construction.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally: a. At the site, employees were exposed to fall hazards during concrete wall construction, in that the tubular welded frame scaffold used was not braced on both sides. A single frame high scaffold with a total of 26 frames had the cross bracing from the one side missing.
