As observed on or about 02/01/2013, employees working at the dough mixing machine were exposed to laceration/puncture hazards or eye injuries associated with compressed air that was conveyed through PVC piping. The compressed air was stated to be 90psi.

No guardrails were in place for when the door was opened, and employees opening the door were standing at the floor edge of the maintenance shop exposed to the fall.  Employees had to open the door to allow air flow through the shop while welding in the shop.
