29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) The ladder placed in front of the building used by the employee to gain access to the roof was not positionded the required minimum of 3 feet above the landing or eave of the roof, on or about February 7, 2013.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and stairways had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders: (a). The employee was not trained on ladder safety and the correct use of an extensioin ladder used to gain access to the roof of a two story residence under construction, on or about February 7, 2013.
