29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   a) U.S. Department of Veterans Affairs-In the Canteen Kitchen-Employees were using the Ecotemp liquid soap, classified as a corrosive, in the dishwasher.  The employee(s)  were not required or provided protective clothing such as apron, or rubber boots in the event of a splash, on or about February 7, 2013.

29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) U.S. Department of Veterans Affairs, Decatur, GA-In the Canteen Kitchen, an employee is required to transfer Ecotemp liquid soap from one container to another, without using splash resistant goggles in the event of a splash on or about 2/7/13. The Ecotemp is classified as a corrosive. The VA provided safety glasses to one employee, however, there is also at least one other employee who sometimes have to transfer the Ecotemp when the main employee is not available or on leave.  The use of splash resistant goggles was not ensured.   b) U.S. Department of Veteran Affairs, Decatur, GA-In the Histology Lab, employees add formalin to containers with tissues received from the operating rooms.  The VA had provided splash resistant goggles to employees, however, some old ones were still being used on or about 2/7/13.  The use of splash resistant goggles was not ensured.   The U.S. Department of Veteran Affairs, Decatur, GA was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.133(a)(1), which was contained in OSHA inspection number 533920, notice number 1,  item number 2, and was affirmed as a final order on 10/24/2012, with respect to a workplace located at 1670 Clairmont Road, Decatur, GA 30033.

29 CFR 1910.138(a): The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s)   a) U.S. Department of Veterans Affairs, Decatur, GA-In the Canteen Kitchen, an employee is required to transfer Ecotemp liquid soap from one container to another, without using corrosive resistant gloves, on or about 2/7/13.    The VA provided hand protection to one employee, however, there is also at least one other employee who sometimes have to transfer the Ecotemp when the main employee is not available or on leave.   The U.S. Department of Veteran Affairs, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.138(a), which was contained in OSHA inspection number 533920, notice number 1, item number 3 and was affirmed as a final order on 10/24/2012, with respect to a workplace located at 1670 Clairmont Road, Decatur, GA.

29 CFR 1904.29(b)(3): Each recordable injury or illnesses was not entered on the OSHA 300 log and/or incident report (OSHA Form 300A or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:   a)U.S. Department of Veterans Affairs, Decatur, GA-The employer did not record each injury or illness that resulted in lost work days on the OSHA 300 Log with a check in column H (Days Away from Work) and the actual number of days in Column K and/or Column L (On the Job Transfer or Restriction) from 1/1/13-2/7/13 in the following three cases:   1. An employee was injured on January 8, 2013, which resulted in 24 days away from work-case #2013-00120.   2. An employee was injured on January 21, 2013, which resulted in 10 days of job transfer or restriction-case #2013-00158.   3. An employee was injured on January 24, 2013, which resulted in 1 day away from work-case #2013-00157.   The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.7(b)(3), which was contained in OSHA inspection number 314252914, notice number 2, item number 1 and was affirmed as a final order on 05/26/2010, with respect to a workplace located at 1670 Clairmont Road, Decatur, GA.
