29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems or an alternative fall protection measure under another provision of 29 CFR 1926.501(b), nor did the employer develop and implement a fall protection plan which met the requirements of 29 CFR 1926.502(k):    This violation occurred on February 7, 2013, at a workplace located at 5435 Gaston Ave., Dallas, Texas 75214; where at least six (6) employees, who were observed working from the roof, were not protected from falling to the lower levels.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:    This violation occurred on February 7, 2013, at a workplace located at 5435 Gaston Ave., Dallas, Texas 75214; where the side rails of the extension ladder employees used to access the roof did not extend at least 3-feet above the upper landing surface.
