29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a). Worksite located at 820 W. Liberty Drive, Liberty, Missouri- Employees were observed working and walking beneath overhead hazards and were not wearing any means of head protection.

29 CFR 1926.1053(a)(8): A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used: (a). Worksite located at 820 W. Liberty Drive, Liberty, Missouri- Employee was exposed tof a fall hazard in that the employee had collapsed his metal spreader and leaned the ladder against a support beam. (b). Worksite located at 820 W. Liberty Drive, Liberty, Missouri- An employee was misusing his step ladder in that he had one leg on the ladder and one on the top of door header which could cause the ladder to tip over due to the force applied by the employee.
