29 CFR 1910.179(n)(2)(i): The hoist chain or hoist rope shall be free from kinks or twists and shall not be wrapped around the load. a. Truck Repair Shop/Third Bay/Section - On or about February 12, 2013, the employer's overhead hoist chain # 3 ( JET 1/2 Ton L - 80 with # 4038) missing its hook mechanism was altered with another metal chain piece to be wrapped around the load. Employees were thereby exposed to struck by hazards from falling loads.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. a. Truck Repair Shop - On or about February 12, 2013, portable fire extinguishers were not mounted and identified. The specific locations and their conditions found were the following; 1. In the Parts Storage Room's Floor - a portable fire extinguisher was not mounted and identified as such. Employees were thereby exposed to fire hazards. 2. Between Bay # 1 and Bay # 2 - a portable fire extinguisher was not identified. Employees were thereby exposed to fire hazards. 3. Bay # 2/Computer's Kit - a portable fire extinguisher was not identified. Employees were thereby exposed to fire hazards. 4. Northeast Corner - a portable fire extinguisher was missing from its identified location. Employees were thereby exposed to fire hazards.
