29 CFR 1910.146(d)(2): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not identify and evaluate the hazards of permit spaces before employees entered: On or about February 6, 2013, employees of Jacobs Services of North America, Inc. entered the #2 Triple 2nd Effect Separator at Archer Daniels Midland Company and the overhead hazards associated with built-up corn material and falling debris were not identified and evaluated prior to entering the permit-required confined space.

29 CFR 1910.146(f)(14): The entry permit that documented compliance and authorized entry to a permit space did not identify any other information whose inclusion was necessary, given the circumstances of the particular confined space, in order to ensure employee safety. On February 6, 2013, employees of Jacobs Services of North America, Inc. entered the #2 Triple 2nd Effect Separator, a permit-required confined space, at Archer Daniels Midland Company to erect a scaffold and the entry permit did not contain or identify information related to overhead hazards due to material build-up.
