29 CFR 1910.26(c)(1): Good safe practices in the use and care of ladder equipment were not employed by the users to get maximum serviceability, safety, and to eliminate unnecessary damage of equipment: (a) On or about 02/07/13, 2803 Pinson Valley Parkway, Birmingham, AL 35217, an employee was descending the portable ladder while carrying a large container in one hand. (b) On or about 02/07/13, 2803 Pinson Valley Parkway, Birmingham, AL 35217, the 24 ft. portable extension ladder was placed in the driveway where it could be accidentally displaced.
