29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (a) At Refrigerant Recycling, Inc. employees were provided and required to wear respirators due to the dust generated when handling appliances, but the employees were not given a medical evaluation to determine their ability to use the respirator.

The brake pedal was missing the anti slip brake pad cover.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: (a) At Refrigerant Recycling, Inc. on or about February 8, 2013 an electrical panel box located in the "F" Storage Area had two openings on the side of the boxe that were not effectively closed; (b) At Refrigerant Recycling, Inc. on or about February 8, 2013 an octagon box on the wall in the reclaim area had a 3/4" diameter opening on the side of the box that was not effectively closed; and (c) At Refrigerant Recycling, Inc. on or about February 8, 2013 a panel box in the Oil Contamination Area on the wall had an opening on top 1 1/2 inches in diameter that was not effectively closed.
