29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: (a) On February 6, 2013, at the facility located at 1528 E Trafficway, Springfield, MO., the Case 585 E Powered Industrial Truck located in Pipe Fabrication Shop was missing the right fork Safety Pin used to hold the forks in place. Employees were exposed to serious injuries in the event of equipment or load malfunction due to the missing pin. Abatement certification is required for this item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
