29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    a. At the side, employees were exposed to possible cave-ins, in that, a ladder or other means of egress was not provided to enter and exit an excavation 22 feet by 18 feet and up to 9 feet deep. Employees were getting in and out the excavation at the west wall which was 60 degrees and 6 feet deep.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:    a. At the site, employees were exposed to possible cave-ins, while in an excavation 18 feet by 22 feet and up to 9 feet deep with the spoil placed right at edge of the east, north and south walls.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraphs (b) or (c) of this section:    a. At the site, employees were exposed to possible cave-ins while in a trench excavation 18 feet by 22 feet and 9 feet deep without sloping or any other shoring system used to protect them from the nearly vertical walls.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:    a. At the site, employees were exposed to possible cave-ins, in that, an excavation 18 feet by 22 feet and 9 feet deep was not inspected by a competent person knowledgeable with trench excavation safety requirements. Employees were allowed to enter an excavation with nearly vertical walls, without a ladder or other means of egress and with the spoil placed right at the edge of the excavation.
