29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. On or about February 12, 2013 - at the above addressed jobsite, employees conducting build out operations inside the quarry shaft were exposed to injuries from stepping into ten (10) inch by ten (10) inch holes which were created by the presence of rebar floor structure, thereby exposing employees to hazards associated with trips and falls.
