Feed Mill - An employee was elevated in a forklift supported man basket that was not secured to the forks thereby exposing the employee to falls of up to 14 feet.

29 CFR 1910.24(f): "Stair treads." Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs. a. Feed Mill - The stairway connecting the second and third levels was not constructed and used with uniform tread and riser height throughout thereby exposing employees to slip, trip and fall hazards of up to 22 feet.

Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23. a. Feed mill operators station - The plug box used at the station was not rated for a Class 2 Division 2 location thereby exposing employees to potential fire hazards. b. Feed mill operators station - The cable tray used above the operators station was not installed and rated for a Class 2 Division 2 location thereby exposing employees to fire hazards.
