29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On February 6, 2013 at the job site located at 1504 NE 96th Street in Liberty, MO employees were exposed to fall from height hazards in that they were accessing and egressing the roof structure without the proper use of their ladder.  In that the ladder side rails did not extend at least 3 feet above the eve of the roof, nor was the ladder secured at the top.
