29 CFR 1910.215(a)(4):     Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    (a)  At Napili Sunset Beachfront Resort on or about January 31, 2013 the workrest on the left grinding stone of the 6" bench grinder located in the maintenance shop was one half inch from the grinding wheel; and    (b)  At Napili Sunset Beachfront Resort on or about January 31, 2013 the workrest on the right grinding stone of the 6" bench grinder located in the maintenance shop was one half inch from the grinding wheel.

The box was corroded and the box cover was left open.
