OSH ACT of 1970 Section (5)(a)(1):   The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a hazard of being struck by fragments of Thermoplastic Polyvinyl Chloride (PVC) plastic pipe ruptures.   At the establishment, PVC schedule 40 plastic pipe was used in exposed above ground locations to transport compressed air for the operation of air-operated tools.

29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o)  a) In the Cut Pad area - the nip points between the power transition pulleys and the belt on the primary air compressor, Ingensoll Rand model 1ST, serial number 30T - 728742 and the backup air compressor, products incorporated, model ACP.C66, serial number 104D6078 or not fully guarded, exposing employees to hazards associated with nip points, on or about February 6 2013.

29 CFR 1910.304(a)(2):   Grounded conductors were attached to terminals or leads so as to reverse designated polarity:  a) In the maintenance shop  employees using a Challenger left, model 37000 and serial number 371150, were exposed to electrical hazards in that the electrical system had reversed polarity, observed on or about February the sixed 2013.  b) In the dismantle shop  employees using a Router left, model SPOA1ON700  and serial number CRZ07J0139, were exposed to electrical hazards in that the electrical system had reversed polarity, observed on or about February the sixed 2013.
