29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. 2678 Johnston St. (Albertson's), Lafayette, LA: On or about February 8, 2013, the employer failed to ensure that employees working from a framed scaffold three bucks high were provided with a ladder or other suitable means of access and egress. Employee(s) were using the scaffold end frames to gain access to the upper levels. Employees were exposed to a fall hazard of approximately 18 feet.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted. 2678 Johnston St. (Albertson's), Lafayette, LA: On or about February 8, 2012, the employer did not ensure that employees used fall protection while working on a flat roof performing masonry work to Albertson's store front sign. The ground to eave height was approximately 25 ft., exposing employees to the hazard of falls from elevation.
