29 CFR 1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures: a) Employees were exposed to electric shock hazard using home-made extension cord metal outlet boxes. The metal boxes do not fully enclose the internal live electric parts including the power outlet terminal screws and the employer does not have assurance that the metal box is grounded to its internal wiring; the boxes did not have bushings to prevent damage to the power supply cord where it enters the box.
