29 CFR 1910.157(e)(3): The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request. On or about February 7, 2013, the employer failed to ensure that the portable fire extinguishers were given their annual maintenance check. An extinguisher located in the front shop area and one located along the west wall of the shop were last inspected in May 2011. Employee(s) were exposed to a fire hazard.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. On or about February 7, 2013, the employer did not ensure that guards were installed on the following: a) Acra Lathe (15" chuck), model #2290, serial #0411920 b) Mazak Lathe (12" chuck), model #43760 c) Lathe (15" chuck), model #2290, serial #CHO8931 d) Kingston Lathe (22" chuck), model #H1-29120, serial #202909337 e) Mazak Lathe (20" chuck), serial no. unclear. Employees are exposed to coming in contact with the rotating chuck.

29 CFR 1910.215(a)(4): Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion. Machine Shop (west wall): The clearance between the abrasive wheel and work rest was 1/2" on the right side of the Dalton grinder (model 2LKR9). Employee(s) were exposed to the hazard of being struck by flying objects.

Machine Shop (west wall): On a Dayton 8" grinder, the clearance between the abrasive wheel and the metal housing was 3/4" on the left and 7/8" on the right. Employee(s) were exposed to the hazard of being struck by flying fragments in the event the abrasive wheel shattered during grinding operations.
