29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a- 75 Lowell St., Carteret, NJ - On or about 02/12/2013, employees were observed repairing roof shingles and replacing sheeting on a 10/12 pitched roof with an eave height of approximately 24' to the ground without fall protection.

29 CFR 1926.1053(b)(1): Portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. a- 75 Lowell Street Carteret, NJ - On or about 2/12/13 - The ladder provided was not secured and did not extend 3 feet above the working surface of the roof to allow for safe access on and off the roof.
