29 CFR 1926.24: The employer shall be responsible for the development and maintenance of an effective fire protection and prevention program at the job site throughout all phases of the construction, repair, alteration, or demolition work. The employer shall ensure the availability of the fire protection and suppression equipment required by Subpart F of this part. Jobsite: No extinguishers were located in flammable storage areas (gas storage/oxy/aceteylene areas) along with inside heavy equipment such as PW320/ or the two excavators on site.

29 CFR 1926.251(a)(6): "Inspections." Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. Jobsite: Employees were using defected rigging hooks on chain rigging throughout the project with no safety latches or daily inspections being done.
