29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:    a)      On 2/13/2013, the employee was utilizing full-face respirators without having a respiratory program written and implemented at the facility.       All provisions of 29 CFR 1910.134 (c) must be covered in a respirator program. Key elements include, but not limited to, the following:        1.    Procedures for selecting respirators for use in the workplace,      2.    Medical evaluations of employees required to use respirators,      3.    Fit testing procedures for tight-fitting respirators,      4.    Procedures for proper use of respirators,      5.    Procedures and schedules for cleaning, storing, inspecting, repairing, and maintaining,      6.    Procedure to ensure adequate air quality,      7.   Training of employees in the respirator hazards and proper use,      8.    Procedures for regular evaluating the effectiveness of the program          ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.134(e): The employer did not provide a medical evaluation for respirator use:  a)      On 2/13/2013, the employee utilized a full-mask respirator without having medical evaluation performed. Respirators may place a physiological burden on employees that varies with the type of respirator, job and workplace condition, and medical status of the employee. A medical evaluation must be performed as 29 CFR 1910.134 (e) for those employees wearing respirators.   ABATEMENT CERTIFICATION REQUIRED.
