On or about 2/12/13, Tank 24, An air testing instrument was not properly maintained in that it was not properly zeroed and calibrated and was used for air monitoring of Tank 24 on 2/7/13, 2/8/13, and 2/12/13 for employee entries to remove vegetable oil which exposes employees to oxygen deficiency hazards, burn hazards, and slip and trip hazards.

On or about 2/7/13, Tank 24, During employee entry operations, continuous air monitoring was not conducted for Tank 24 which had steam piping that was not isolated from Tank 24.  An employee worked inside Tank 24 to remove vegetable oil and was exposed to oxygen deficiency hazards, burn hazards, and slip and trip hazards.
