29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing electrical hazards) a) The employers safety and health program did not provide for a competent person to inspect the jobsite for electrical hazards and allowed employees to work near to a live 460 AC voltage over head service line. Employees engaged in re-roofing operation on a commercial flat roof structure worked under and within 6 inches of that electrical shock potential.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) Employees engaged in re-roofing operation on a commercial flat roof structure were allowed to work under and within 6 inches of the live 460 AC voltage over head service line entering down through the roof. The service line was not de-energized or insulated to effectively protect employees from the electrical shock potential(s).
