29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use (a) Ammonia engine room area - On February 12, 2013 the accumulator recirculator tank did not comply with the International Institute of Ammonia Refrigeration ANSI/IIAR 2-2008 paragrpah 14.3.1 in that the accumulator insulation was removed and there was condensation on the tank.
