The flexible conduit had been pulled away, observed on or about 2/13/13. (b) Pallet Services Inc., 66 Pennsgrove-Pedricktown Road, Pedricktown, NJ, Saw area, Hawk Sidewinder/Dismantling Saw: The GE General Duty Safety Switch was missing a knockout on the top of the box in the middle, observed on or about 2/13/13. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
