The north end of the dock measured 49 inches above grade, observed on or about 2/13/13.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:    (a) Pallet Services Inc., 66 Pennsgrove-Pedricktown Road, Pedricktown, NJ: The employer failed to provide training on the use of fire extinguishers and the hazards associated with fighting an incipient stage fire to employees who were expected to use a portable fire extinguisher to fight an incipient stage fire. Occurred on or about 2/13/13.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

Conductors entering boxes, cabinets, or fittings were not protected from abrasions:    (a) Pallet Services Inc., 66 Pennsgrove-Pedricktown Road, Pedricktown, NJ, Saw area: The wires entering the GE General Duty  safety switch box to the Hawk, sidewinder/dismantling saw, were not protected from damage.  The flexible conduit had been pulled away, observed on or about 2/13/13.    (b) Pallet Services Inc., 66 Pennsgrove-Pedricktown Road, Pedricktown, NJ, Saw area, Hawk Sidewinder/Dismantling Saw:  The GE General Duty Safety Switch was missing a knockout on the top of the box in the middle, observed on or about 2/13/13.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
