29 CFR 1926.152(e)(4): Units dispensing flammable or combustible liquids were not protected against collision damage:    a) Jobsite, Six Mile Run Area VFD, 1100 Six Mile Run Road, Six Mile Run, PA:  On or before 02/12/13, a manual dispensing unit on 275 gallon diesel tank located adjacent to the job trailer was not protected against collision damage. The dispensing unit and tank were used to refuel a Gehl RT forklift, RS 8-42.    b) Jobsite, Six Mile Run Area VFD, 1100 Six Mile Run Road, Six Mile Run, PA:  On or before 02/12/13, a manual dispensing unit on a 36 inch diameter by 5 feet long kerosene tank was not protected against collision damage. The tank was  located adjacent to rear of the building under construction and a truck crane with a personnel platform used by a sub-contractor was operating in the immediate area.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    a) Jobsite, Six Mile Run Area VFD, 1100 Six Mile Run Road, Six Mile Run, PA:  On or before 02/12/13, two (2) employees working inside a personnel platform fabricated by the employer installing siding material which was elevated approximately 4 feet from the ground level by a rented Gehl RT forklift, RS 8-42. The personnel platform was not approved for use by the forklift truck manufacturer and was not firmly secured to the lifting carriage and/or forks
