29 CFR 1910.134(e)(1): The Employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or was required to use the respirator in the workplace.     a) Revent Corp., Piscataway, NJ site - Welding Areas: Employees, who were required to wear respirators such as, but not limited to N95 filtering face-piece respirators with exhalation valve (3M 8516) and/or half face respirators such as, but not limited to MSA 00817664 with P100 cartridges, 3M 7502 with organic cartridges, etc.  were not provided medical evaluations prior to the use of the respirators in the workplace.     Condition noted on/or about February 11, 2013.
