29 CFR 1910.212(a)(5):   Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.    Fire Fighting Equipment Locker - The guard to the Taskmaster heater fan had 2" openings.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.    In the following instances; suitable facilities for quick drenching or flushing of the eyes and body were not immediately available due to hazard creating incumbrances.      a)   Cooling Tower -  A 6" high dike surrounding the 500 gallon tank of sodium hypochlorite 12.5% area was an incumbrance to the emergency shower and eyewash.     b)   Water Treatment - The path from the RO system and caustic soda tank had ground incumbrances with handles sticking up from the floor cover plates.

29 CFR 1910.178(q)(7):    Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.    In the Yard - The rear floodlight on the Toyota 7785 forklift did not work.

29 CFR 1910.303(b)(1)(vi):    Electrical equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees.  Arcing effects shal be taken into consideration in determining the safety of the equipment.    Maintenance Department - The plug attached to the Milwaukee 14" abrasive cut-off saw, serial #98242814, had been damaged by the arcing effects between the receptacle and ungrounded prong.
