29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift:    a)  Brookover and Son Construction on or about February 13, 2013, at the address of 106 Michaels St. Bettsville, OH: The employer failed to ensure an employee working from an aerial lift was adequately protected from falls.  An employee working from the lift installing fascia work was not clipped off to the basket while the platform was elevated. The employee was exposed to an approximately 30 ft. fall to the ground below.
