29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. 1. Inside of the welding area, near the south wall, employees were using a duplex metal junction box with knockouts as an extension cord to energize a pedestal fan. The pedestal fan was plugged into the duplex metal outlet box.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. 1. In the production area, on the west wall, there was a large knockout missing underneath the 110/220 volt electrical panel box. 2. In the production area, on the south wall, there was a knockout missing underneath the 110/220 volt electrical panel box.
