Main Plant Dock, Crane Number 2; on or about 7 February 2013 and times prior thereto, an Aurora Crane, Model 30 K 4000, Serial Number 593-0705-90; the flexible cord connected to the heater, was not connected to a device and fitting so that strain relief was provided. This condition exposed employees to an electrical shock hazard.
