29 CFR 1910.37(a)(4): Safeguards designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting) must be in proper working order at all times. 1. Inside of the main product cooler, there were two (2) illuminated exit signs hanging down by their wires next to the large blowers. Each exit sign was hanging by two (2) small wires that energizes the sign. The exit signs are heavy and could have fallen and struck employees on their heads.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. 1. At the packing station, on the west wall, employees were using a metal scale (# 2) that was not grounded. The ground pin was missing from the plug located on the electrical cord used to energize the weighing scale.
