29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    On or about February 13, 2013 and times prior thereto, employer did not effectively lockout equipment when removing a guard to clear equipment.  In the mixing room employees remove the expanded metal screen from the hopper to clear clogged material without applying a lock to the energy isolating device exposing them to amputation hazards in the event of an unexpected startup.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about February 13, 2013 and times prior thereto, employees were exposed to rotating parts and ingoing nip points in the following instances:    instance a.) The South Bend Lathe in the maintenance area was not equipped with a chuck guard exposing employees to the rotating chuck and jaws.    instance b.) The end of the mesh chain conveyor where it exits the freezer in the pack-out area has an exposed ingoing nip point where it passes over the end roller exposing employees to caught-in injuries.
