Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6.    a.) Auxiliary Systems Inc. On or about 02/11/13, the NOVAX Class 1 rubber gloves used during testing of live conductors were not provided with an electrical test prior to initial use and every 6 months thereafter.

29 CFR 1910.304(b)(3)(ii)(B):     Receptacles other than 125 volt, single-phase, 15-, 20-, and 30-ampere receptacles that are not part of the permanent wiring of the building or structure and that are in use by personnel shall have ground-fault circuit-interrupter protection for personnel.    a.)  ACU-4 Hanger Bay 3 LCAC-86: On or about 02/11/13, ground fault interrupter protection was not provided to employees while using an orange drop light in that the drop light was plugged into a temporary receptacle outlet on a Miller 350 welding unit.
