Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6. a.) Auxiliary Systems Inc. On or about 02/11/13, the NOVAX Class 1 rubber gloves used during testing of live conductors were not provided with an electrical test prior to initial use and every 6 months thereafter.

ACU-4 Hanger Bay 3 LCAC-86: On or about 02/11/13, ground fault interrupter protection was not provided to employees while using an orange drop light in that the drop light was plugged into a temporary receptacle outlet on a Miller 350 welding unit.
