29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: Employee(s) performing the duties of entry supervisor had not received training over the confined space entry program including the duties of the entry supervisor such as, but not limited to the following: knowing the hazards the authorized entrant may be exposed to, verifies by checking that the appropriate entries have been made on the permit, terminates the entry and cancels the permit, verifies that rescue services are available and the means for summoning them are operable, removes unauthorized individuals who enter or attempt to enter the permit space during entry operations, determines that the entry operations remain consistent with the terms of the entry permit and that acceptable entry conditions are maintained, and etc., thus exposing the authorized entrants to asphyxiation hazards.
