29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: The employer failed to ensure that employees conducting forklift refueling operations followed the Job Safety Analysis procedures developed by the company. A JSA was developed for use companywide. A worker received second degree burns to his hands and arms when propane was shot up his coat sleeve. The gloves the employee was wearing did not cover his sleeve to preclude the propane from going up the sleeve.
