McArthur, Irving, TX 75038; where at least one (1) employee, who was working and walking underneath overhead work, was not protected from head injuries from falling tools and materials by wearing hard hat.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    This violation occurred on February 14, 2013, at a workplace located at 5550 N. McArthur, Irving, TX 75038; where at least one (1) employee worked from an aerial lift without wearing body belt and a lanyard attached to the boom or basket.       The Kevco Electric, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.67(c)(2)(v), which was contained in OSHA inspection number 316043983, citation number 1, item number 1 and was affirmed as a final order on 1/06/12, with respect to a workplace located at 976 Keller Pkwy, Keller, TX.
