29 CFR 1910.110(h)(14):     LP gas service station(s) were not provided with at least one approved portable fire extinguisher having at least an 8-B, C rating:      On or about February 11, 2013, and at times prior thereto, at the propane storage tank areas the employer allowed employees to refill propane from a big storage tank to smaller cylinders without having at least one approved fire extinguisher provided in the areas.  Employees were exposed to fire hazards.

On or about February 11, 2013, and at times prior thereto, in the service areas an electrical extension cord was observed being damaged and run under a metal vending machine.  The employees were exposed to electrical shock hazards.
