29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: This violation occurred on February 14, 2013, at a workplace located at 6822 St Francis Ct., Irving, TX 75062; where at least six (6) employees, who were working on the roof, were not protected from falling to the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: This violation occurred on February 14, 2013, at a workplace located at 6822 St Francis Ct., Irving, TX 75062; where at least six (6) employees used ladders to access/egress the roof and the ladders side rails did not extend at least 3 feet above landing level.
