29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected. M/V Gordon Jensen, Machine room; on or about 11 February 2013 and times prior thereto, the employer did not ensure that a Miller Arc Welder, Model # SRH 222, Serial # 72-646394, electrode holder cable was free of repairs. The electrode cable was damaged and covered with tape within ten feet; approximately 24 inches from the end of the cable. This condition exposed employees to an electrical shock hazard.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. M/V Gordon Jensen; on or about 11 February 2013 and times prior thereto; two instances of electrical hazards exposed employees to electrical hazards: (a) Factory hydraulic room; the employer did not ensure that unused openings in the electrical boxes and panels were effectively closed. 208/120V Electrical panel 99 had four open blanks. Panel blanks numbers 26, 28, 38 and 40 were open. This condition exposed employees to electrical shock and burn hazards. (b) Main engine room; an electrical drop light with metallic shield; make and model indiscernible; was missing the grounding plug. This condition exposed employees to electrical shock and burn hazards. Icicle Seafoods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 307504043, citation number 1, item number 1, and was affirmed as a final order on 4/02/2009, with respect to a workplace located at Dutch Harbor, Alaska.
