29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: M/V Gordon Jensen Refrgieration System: On or about February 11, 2013 and at times prior thereto, valve identification tags were missing at multiple locations of the refrigeration system. This included but was not limited to: 1. Oil drains, Sabroe compressors; 2. Heat Exchanger; 3. Cornell Liquid Pumps relief vent lines; 4. LPR Oil Pots -40 & -15, drain valves; 5. Sabroe Oil Filter housing; 6. Sabroe compressor transducers; 7. Mycom oil filter; 8. Mycom oil drain; 9. Vilter compressor oil filter transducers; 10. Port-side external Blast Freezer; 11. Starboard-side plate freezer zone 212; 12. Control Pressure Receiver drain valve and bullet glass level isolator; and, 13. North Star ice machine vapor suction and liquid supply. Valve identification was not being maintained as required by the Process Safety Management (PSM) program which incorporated by reference ASHRAE 15 and ASHRAE 26. This condition exposed employees to inhalation and eye and skin burn hazards in the event of an uncontrolled release of anhydrous ammonia. Icicle Seafoods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 307506535, citation number 2, item number 1 and was affirmed as a final order on September 22, 2010, with respect to a workplace located at P/V Arctic Star, Clark's Point, Alaska.
