29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: a) Jobsite corner of North Main and Walnut Streets, Washington, PA: On or about February 15, 2013, during roof repair/patching activities on a slate/tile roof determined to have an approximate 8/12 pitch, the employer did not ensure that employees used fall protection systems/devices where exposed to falls of up to approximately 30 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Jobsite corner of North Main and Walnut Streets, Washington, PA: On or about February 15, 2013, during roof repair/patching activities on slate/tile roof with an approximate 8 1/2 pitch, the employer did not ensure that employees had the ladder used to gain access to/egress from the roof extended at least 3 feet above the roofline or that it was secured and a grab bar was provided, exposing employees to fall hazards of up to approximately 30 feet.
