On, or before, February 14, 2013, Employees were exposed to an electrical hazard as a flexible cord (extension cord) was secured to the building structure from an electrical outlet in 3 Dock room to a permanently positioned piece of equipment from the ceiling.

29 CFR 1910.305(a)(2)(ix): Lamps for general illumination in 3 Dock were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. On or on dates prior to February 14, 2013 employees working at Nor Am Ice and Cold Storage in 3 Dock were exposed to an electric shock hazard in that the lamps used in the area were not protected by a guard.
