29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a)  Jobsite, Bob Massie Toyota, North Huntingdon, PA:  On or about 02/15/13 employees working in JLG aerial lifts were not wearing a harness with a lanyard attached to the boom or basket.  The potential fall hazard to the employees was approximately 24 feet to the ground.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:    (a) Jobsite, Bob Massie Toyota, North Huntingdon, PA:  On or about 02/15/13 employees engaged in a steel erection activity, laying decking, who were approximately 24 feet above a lower level, were not protected from falling by the use of guardrail systems, safety net systems, personal fall arrest systems, positioning devices systems or fall restraints in accordance with the requirements of this standard.

29 CFR 1926.454(c)(3): The employer failed to retrain affected employees who were working with scaffolds that showed indications through work performance of not retaining the requisite proficiency:    (a) Jobsite, Bob Massie Toyota, North Huntingdon, PA:  On or about 02/15/13 the employer failed to retrain employees who were working from aerial lifts without adequate fall protection which indicated the employees had not retained the requisite proficiency of working safely from scaffolds.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program:    (a) Jobsite, Bob Massie Toyota, North Huntingdon, PA:  On or about 02/15/13 the employer did not train all employees engaged in steel erection activities that were exposed to fall hazards of approximately 24 feet in accordance with the requirements of this section.  Employees were performing steel erection activities and were utilizing a "safety monitor" as a method for fall protection which is not a viable option under this section.
