On or about February 13, 2012, at the above addressed jobsite, employees were inside of aerial lifts without using a body belt with lanyard attatched to the boom or basket.           b.  On or about February 13, 2013, at the above addressed jobsite, employees were inside of aerial lifts without using a body belt with lanyard attatched to the boom or basket.
